PATIENT ELIGIBILITY & RESPONSIBILITIES

• No health insurance

• Live in the Austin area

• TEXT 512-768-8609 to make an appointment. A scheduler will be in contact within 1 business day.

• TEXT 512-768-8609 to cancel appointment if you are unable to come. Please cancel at least 24 hours ahead whenever possible.

• Patients are encouraged to make a $10 donation at each visit, but no one will ever be turned away for inability to donate.

• Patients who arrive more than 15 minutes late for an appointment may need to wait to be seen or may be asked to reschedule.

• Children can be seen. No childhood immunizations are offered.

• Acute primary care for common illnesses

• Chronic disease care

• Prescription medications

• Over the counter medications

• Basic lab work

• Basic imaging

• Flu Shots

• Medical supplies

• Nutrition education

• Well-woman exams

• Preventative care

• School physicals (bring school paperwork)

• Limited physical therapy
